Few Tips to Ensure a Smooth First Appointment

If this is your first appointment with Bloomingdale Foot & Ankle, there are some things you can do to help it go smoothly. Please arrive about 15 minutes earlier than your scheduled appointment so that you'll have enough time to fill out your welcome paperwork.

To get a head start on this process, we do encourage you to print out and fill out the two forms below and bring them with you to your appointment.

What You'll Need to Bring on Your First Visit

  • Current health insurance card
  • A picture ID
  • Listing of your current medications
  • Important medical/surgical history documents - including X-ray films, CDs, and reports

Insurance and Payment Options

At Bloomingdale Foot & Ankle, we accept most health insurance plans. As a token of our gratitude for choosing us for your care, we will waive the insurance co-payment for your first appointment. Our clinic bills to most insurance companies* including Medicare and Blue Cross Blue Shield.

We also accept cash, checks, and major credit cards. If necessary, payment and financing options are available as well. 

If you have any questions before your first visit, please give us a call. Our friendly staff will be happy to help you out the best that we can!

*Please note that we do not accept Medicaid. This is different from Medicare.
